What to Bring to Your Oral Surgery Consultation

To help your visit go smoothly, please bring:

  • Your referral slip and any existing x-rays (if available)

  • A current list of medications you are taking

  • Your medical and dental insurance information (completed forms, if possible)

Important: Patients under 18 years of age must be accompanied by a parent or legal guardian.


IV Anesthesia & Pre-Surgical Requirements

If your procedure involves IV anesthesia, a pre-operative consultation and physical exam are required.

Please remember:

  • Do not eat or drink anything for 8 hours before surgery

  • You must have a responsible adult family member or friend bring you to your appointment and take you home afterward
